Slave owners have a long term interest in the productivity of their slaves, so they have incentive to keep them healthy and motivated. Democratically elected rulers have no long term interest, so their incentive is to milk the subjects for all they can squeeze out of them while in power, with no regard for their long-term productivity. Of course, they will be limited by the fact that they need to maintain their legitimacy in the public eye.
